Strive Health is hiring a Clinical Lead Nurse Practitioner to manage a panel of patients with chronic heart failure, chronic kidney disease, end-stage kidney disease, and related conditions. The role provides longitudinal in-person, telehealth, and dialysis-clinic care; coordinates with external providers; manages care plans, diagnoses, HCC coding, HEDIS gaps, and transitions; and supports shared decision-making and advanced care planning. The position requires a nursing master's degree, at least two years of relevant family or primary care nurse practitioner experience, EHR experience, current licensure, national board certification, DEA eligibility, and BLS or CPR certification. Compensation is $112,000–$140,000 annually plus a target 10% bonus, with hybrid-remote flexibility and no visa sponsorship.
