McGregor PACE is hiring an Adult/Geriatric Nurse Practitioner to provide primary care and comprehensive health services to frail seniors in its Brooklyn and Elyria office locations. The role collaborates with an interdisciplinary team, manages acute and chronic illness, supports preventative and wellness care, coordinates outpatient follow-up, and may precept nursing staff or students. It requires a master-prepared nurse with Family or Adult/Gerontology Nurse Practitioner certification, an active unrestricted Ohio license, at least two years of direct care experience with older adults, at least two years of advanced practice nursing experience, and BLS certification.
