VNAcare is hiring a per diem Nurse Practitioner for its Rancho Cucamonga location and surrounding areas during the day shift. The role provides hospice care to critically, chronically, or terminally ill patients, including home visits, patient assessment, documentation, interdisciplinary collaboration, and recertification scheduling. Applicants must hold current California Registered Nurse and Nurse Practitioner licenses and at least one year of professional nursing experience; hospice or palliative care experience is preferred. The position offers flexible schedules, mileage reimbursement, orientation and on-the-job training, and $75.00 per hour.
