Description
Vynca is hiring a Nurse Practitioner to provide evidence-based clinical care to complex and seriously ill patients through telemedicine and in-person visits across northern New Jersey and other states. The role includes symptom management, care planning, diagnostic and therapeutic testing, prescribing, goals-of-care and advance-care-planning discussions, transition-of-care coordination, hospice facilitation, interdisciplinary collaboration, and after-hours on-call coverage. Candidates must have active New Jersey RN and NP licenses, at least two years of experience caring for complex, seriously ill patients, ANP/AGNP/FNP certification, and graduate-level NP education; hospice or palliative care and home health experience are preferred.
