Cadence is hiring a fully remote Nurse Practitioner to serve as the primary clinical point of contact for patients in remote chronic-care programs. The role focuses on guideline-directed medical therapy for heart failure, type 2 diabetes, and hypertension; medication titration; clinical escalations; synchronous and asynchronous care; and collaboration with physicians, registered nurses, and patient-success teams. Candidates must hold an active compact multi-state RN license, preferably NP licensure in California, Washington, or Arizona, and have at least five years of chronic-disease experience and three years of cardiology experience. The position offers an annual salary of $130,000–$160,000, plus potential incentive compensation and benefits including medical, dental, and vision coverage.
