St. Joseph's Center is hiring a Nurse Practitioner - Clinical Care Coordinator to provide on-site clinical leadership and oversight for nursing services in its ICF Programs. The role supervises nursing staff, conducts resident evaluations, manages medication orders, ensures CMS and Pennsylvania ICF/ID regulatory compliance, supports quality improvement and survey readiness, and collaborates with interdisciplinary teams. Candidates must hold an accredited degree, a valid Pennsylvania Nurse Practitioner license, at least two years of supervisory experience, and three to five years of clinical experience; long-term care, ICF/ID, or intellectual and developmental disability experience is preferred. Compensation starts at $103,000, with medical, dental, and vision insurance and other benefits.
