Description
The Hospice Nurse Practitioner functions independently as an advanced nursing professional in a hospice environment, assessing patients, creating and modifying care plans, making referrals, educating patients and families, monitoring status, and evaluating care effectiveness. The role collaborates with medical and nursing staff and interdisciplinary teams to provide physical, emotional, psychosocial, and spiritual care to individuals and families experiencing life-limiting illness. The position requires a nursing degree, a master's in nursing, and five years of recent relevant experience, and is full-time, regular, exempt, and scheduled Monday through Friday with weekends as needed.
