Community Physicians is hiring a full-time Nurse Practitioner in LaSalle County to provide primary and post-acute care for medically complex geriatric patients in assigned AL/IL and skilled nursing facilities. The role includes comprehensive assessments, diagnosis and treatment of acute and chronic conditions, medication management, care coordination, documentation, and participation in quality improvement. The NP will travel daily among facilities in Morris, Streator, Minooka, Sandwich, Dwight, Marseilles, and Plainfield, with mentorship from a fellowship-trained geriatrician and potential growth into clinical leadership. Compensation is $120,000 annually with medical, dental, vision, HSA, paid time off, retirement, life insurance, malpractice coverage, and CME support.
