Conifer Park Outpatient Clinic in Glens Falls, New York is seeking a Nurse Practitioner to provide comprehensive outpatient medical care, including history and physicals, diagnosis, diagnostic testing, medication management, substance-use disorder treatment, emergency evaluation, pre-admission clearance, follow-up, discharge planning, and patient education. The role also includes on-call coverage, multidisciplinary team participation, and performance-improvement activities.
