Description
Optimal Care is hiring a Nurse Practitioner or Physician Assistant to provide primary and consultative care for geriatric and medically complex patients in homes, assisted living facilities, and nursing homes throughout Jackson and surrounding areas. The clinician will conduct evaluations, create treatment plans, prescribe and monitor medications, coordinate with physicians and care teams, and support chronic disease management and prevention of avoidable hospitalizations. The position is Monday through Friday, 8 a.m. to 5 p.m., involves daily travel, and offers a $110,000–$125,000 annual base salary plus monthly bonus potential and comprehensive benefits.
