Description
Community Health Network is hiring an Advanced Practice Provider for its Community ProCare at Home Program in Indianapolis and surrounding counties. The role provides in-home house-call services to patients who have been hospitalized, recently discharged from a skilled nursing facility, or are homebound. Candidates should have advanced nursing practice expertise with chronically ill patients, knowledge of local resources, prior house-call or home-care experience, and the ability to work independently. Indiana licensure and certification requirements are required.
