Form Health is hiring a fully remote, W2 part-time Obesity Medicine Physician for a US-based clinic. The role provides direct telemedicine patient care, treatment planning, medical management, chart review, and prescription of non-controlled obesity medications, with collaboration with advanced practice providers and registered dietitians. Applicants must have an MD or DO degree, ABOM certification, an active unrestricted US medical license, at least two years of post-residency experience in internal medicine, family medicine, or obesity medicine, and Medicare opt-in. The position requires a minimum of 20 hours per week, including 16 hours of clinical patient-facing time and 4 hours of administrative time, and offers a base annual salary of $94,500 to $103,500 USD.
