Description
The Occupational Therapist provides evaluation, management, and treatment services to patients referred by physician departments in a home-health setting. Responsibilities include assessing patients, families, and home environments; developing and revising plans of care; coordinating with physicians, caregivers, and other health-care providers; implementing prescribed services; evaluating patient response; managing cases; documenting care; supervising Home Care Aides and Certified Occupational Therapy Assistants; and participating in quality improvement and professional development.
