St. Joseph's Home Care is hiring an Occupational Therapist for its Integrated Comprehensive Care Program in Hamilton, Ontario. The role is community-based and entry level, focused on assessing, planning, and implementing rehabilitation programs for adults transitioning from hospital to home and clients with complex medical needs. The therapist will conduct holistic assessments, develop individualized treatment plans, educate clients and caregivers, collaborate with interdisciplinary teams, supervise support staff and students, document care, and support quality improvement. Candidates need an OT degree, Ontario registration, current BCLS/HCP certification, 1–3 years of experience, and familiarity with home/community care, mobile work tools, and EMR systems.
