Description
The role provides individualized outpatient support to adults with substance-use or mental-health needs who live independently in the Diepholz region. Duties include home visits, one-to-one counseling, and accompanying care to help clients build a self-determined and independent way of living. The position requires a degree in social work, social pedagogy, or a comparable qualification, training in special-needs education, occupational therapy, or a related field, a class B driver’s license, and willingness to use a personal car for business trips.
