PACE Primary Care Physician at myspss

TRU Community Care is hiring a PACE Primary Care Physician in Lafayette, Colorado, working Monday through Friday from 8 a.m. to 5 p.m. The physician provides diagnosis, treatment, ongoing management, clinic and home-based primary care, palliative care, symptom management, and coordinated care for geriatric PACE participants and hospice patients, with phone-only on-call coverage. The role requires an MD or DO, at least two years of primary care experience, a Colorado medical license, DEA and controlled-substance registration, Medicare/Medicaid credentials, and professional liability insurance; experience with frail or elderly patients is preferred.
