Description
The PACE Social Work Associate supports AltaMed’s interdisciplinary PACE team by coordinating participant services, resolving participant and family questions and complaints, addressing Medi-Cal and Medicare issues, processing involuntary disenrollments, conducting satisfaction surveys, making community-resource referrals, and performing case management activities. The role requires a bachelor’s degree in social work or a related health or human services field, at least two years of customer service or case management experience, knowledge of Medi-Cal, Medicare, and Social Security benefits, and BLS/First Aid certification. Experience with PACE or frail or elderly populations and bilingual ability are preferred.
