PACE Social Work Case Manager at AltaMed

AltaMed is hiring a social work and case management professional to support its PACE (Program of All-Inclusive Care for the Elderly) interdisciplinary team. The role coordinates participant services, handles participant and family questions, complaints, appeals, incident reports, Medi-Cal and Medicare issues, involuntary disenrollments, satisfaction surveys, community referrals, and case management activities while complying with applicable regulations. A bachelor's degree in social work or a related health or human services field, at least two years of customer service or case management experience, knowledge of Medi-Cal, Medicare, and Social Security benefits, and BLS/First Aid certification are required.
