AltaMed is hiring a PACE Social Work Supervisor to oversee social work and other PACE team members at an assigned site. The role supervises staff, coordinates assessments and individualized care plans, supports family conferences and community referrals, monitors documentation and performance, onboards new hires, guides behavioral health and end-of-life planning, ensures regulatory compliance, and collaborates on site safety, staffing, and budgets. An MSW and at least two years of supervisory experience in health-related or human services are required; geriatric case management experience, bilingual ability, LCSW, and BLS certification are also relevant qualifications. The annual salary is $90,090.88–$112,613.60, with medical, dental, and vision insurance among the benefits.
