TRU Community Care is hiring a PACE Medical Social Worker in Lafayette, Colorado, working Monday through Friday from 8 a.m. to 5 p.m. with no on-call requirement. The role provides direct case management for PACE participants and families, including psychosocial assessments, referrals, care-plan coordination, discharge and long-term care planning, advance care planning, mental health and crisis assessment, benefit assistance, and enrollment procedures. The position requires a Master’s Degree in Social Work and at least two years of relevant medical social work experience; experience with frail or elderly populations, individuals with mental health needs, and interdisciplinary teams is preferred. Compensation is $68,580 to $85,812 per year for MSW/LSW holders and $73,354 to $91,750 per year for LCSW holders, with a $6,000 sign-on bonus.
