Description
The Palliative Care Intake RN supports the Comprehensive Care team by triaging referrals and patient or caregiver calls, assigning care to appropriate clinical disciplines and providers, coordinating timely care with primary care or comprehensive care providers, assessing patients and caregivers, identifying care gaps, collaborating on care plans, documenting interventions, and managing Epic in-basket messages. The role is full-time, benefits-eligible, Monday through Friday from 8:00 a.m. to 4:30 p.m., and requires a hybrid arrangement with the employee living in Central Ohio.
