Virginia Garcia Memorial Health Center is hiring a Panel Care Coordinator to support care teams by coordinating scheduled and proactive preventive care for assigned patient panels, especially patients with complex, chronic, or psychosocial needs. The role involves patient communication, medication refill support, registry tracking and data entry, chronic disease management, case management coordination, referrals, and collaboration with pharmacies, insurers, and community providers. Candidates need English and Spanish communication proficiency, healthcare or social-service administrative experience or relevant education/certification, strong organizational, interpersonal, problem-solving, and teamwork skills, and basic proficiency with Microsoft Office and healthcare systems such as Epic and Meditracks.
