Description
One Health Ohio is hiring a Care Coordinator to support medical care delivery by serving as a communication hub for patients, nurses, doctors, and family members. The role handles patient calls and messages, faxes and paperwork, monitor tracking, prescription audits, shot records, lab scanning, referrals, PAP tracking, missed appointments, and electronic health record documentation. It is a part-time position of 16–20 hours per week, Monday through Friday, with benefits including health, vision, dental, life insurance, a 401(k) match, paid time off, and paid holidays.
