Community Physicians is hiring a part-time Nurse Practitioner for a multi-site geriatric Annual Wellness Visit program across the Northern, Southern, or Western suburbs. The role involves conducting comprehensive wellness assessments, cognitive and functional screenings, medication reconciliation, goals-of-care discussions, care-plan development, Chronic Care Management enrollment, and clinical documentation. The position offers 16–32 hours per week, no call, no weekends, and $75 per completed visit, with physician mentorship and a collaborative team-based practice.
