The role involves physical examinations, medical history review, diagnosis, treatment planning, diagnostic testing, medication and supply prescribing, and coordination with patients, families, caregivers, and clinical operations. Candidates must be board certified with a valid Virginia NP or PA certification, have strong clinical and communication skills, and have 1–2 years of primary care experience with geriatric patients; hospice experience is preferred but not required.
