Oswego County Opportunities, Inc. is hiring a Youth Care Manager to provide early intervention and care management services to homeless and at-risk youth at PATH sites in Oswego and Fulton and the OCO Youth Emergency Services (YES!) Shelter. The role includes daily assessments, crisis intervention, advocacy, referrals, case planning, transportation, data tracking, and coordination with community providers, with coverage at the YES! Shelter as needed. The position requires a bachelor's degree in a human services area, 1–3 years of experience with children, youth, families, or a special population, or an equivalent combination of education, experience, and training, along with a valid New York State driver's license and access to a reliable vehicle.
