Description
The Patient Care Coordinator supports the CARDIO4Cities Brunei initiative by re-engaging patients with overdue chronic-disease follow-up care, contacting patients through approved scripts, coordinating appointments with the nursing team, maintaining outreach and appointment records, and escalating clinical concerns. This is a strictly non-clinical, on-site role reporting to the Nurse-in-Charge at designated health centers in Brunei-Muara District, Brunei Darussalam, and requires strong communication, data-management, confidentiality, and professional-conduct skills.
