Uptiv Health is seeking a Full-Time Patient Care Coordinator (PCC) in Troy, Michigan. This role involves assisting with vital collection and documentation, delivering a positive greeting to patients, handling communications from referring providers and patients, managing package deliveries and supplies, and ensuring proper sanitation. The ideal candidate will have strong clinical experience, possess Active CPR Certification, understand clinical processes, and ideally hold a Licensed Medical Assistant (MA) certificate.
