Description
The Chronic Care Management Coordinator provides chronic care management services under the direction of the Manager of Social Services and primary care providers, overseeing up to 250 enrolled patients per month. Responsibilities include patient eligibility assessment, care planning, monthly patient contact, documentation, care coordination, prior authorizations, pharmacy and billing support, and preventive care scheduling. The role requires a high school diploma, at least five years of medical assisting experience, Microsoft and accounting software proficiency, strong communication skills, and completion of required immunizations, PPD screening, and Level 2 background clearance.
