Description
The Patient Intake Coordinator is a front-line public health worker who promotes access to low- or no-cost healthcare programs, conducts outreach to individuals and families, enrolls them in public insurance programs, and provides referrals to health and supportive services. The role processes new-patient paperwork, answers inbound calls, supports clinical teams, maintains data integrity, and serves as a liaison between health and social services and the community. It requires knowledge of the healthcare delivery system, Microsoft Office, a high school diploma or GED, and Level 2 background clearance; a college degree is preferred, and Spanish or Creole communication is a plus.
