Angels of Care Pediatric Home Health is hiring a full-time Pediatric Transitional Nurse, RN or LPN, in Springfield, Pennsylvania. The role provides one-on-one skilled nursing care to medically fragile children and their families during hospital-to-home transitions, including assessments, medication administration, trach and ventilator care, discharge support, caregiver education, interim coverage, and coordination with clinical teams. The position offers day and night shifts, on-call and staffing coverage, local travel, and biweekly pay of $38 per hour, with medical, dental, vision, retirement, and other benefits.
