Blue Ridge Health is hiring a Peer Extender for its Tailored Care Management team. The role supports members’ recovery through outreach, engagement, follow-up, service and appointment coordination, health promotion, referrals, community-resource navigation, case-conference participation, and care-manager support. The position requires a high school diploma or equivalent, Certified Peer Support Specialist certification, lived behavioral health experience, and a driver’s license; prior peer support, community health worker, community navigator, or family navigator experience is a plus.
