Description
AltaMed Health Services Corp. is hiring a Personal Care Attendant II to provide and assist with activities of daily living and instrumental activities of daily living for PACE participants in centers, homes, and virtual care settings. The role also supports care coordination by gathering and reporting participant information, promoting independence and function, and collaborating with interdisciplinary team members on plan-of-care goals. California nurse assistant certification, high school education, BLS certification, and experience with elderly or impaired populations are required or preferred.
