This innovative provincial initiative utilizes a virtual care platform to oversee patient care, offering palliative, complex, and pediatric care for individuals with diverse needs. The role involves delivering in-home support, collaborating with nurses using a digital platform, and ensuring high-quality patient care. The position is permanent, located in the Huron Perth Counties, requiring overnight shifts from 11 PM to 7 AM, including some weekend rotations.
