Description
The Anticoagulation Program Manager/Formulary Manager Clinical Pharmacy Specialist (CPS) or Clinical Pharmacist Practitioner (CPP) manages anticoagulation therapy programs, reviews prior-authorization drug requests, develops therapeutic drug plans, performs medication reconciliation and drug-utilization evaluations, counsels patients, and supports education and quality-improvement activities. The role requires a pharmacy degree, current unrestricted U.S. pharmacy license, English proficiency, and at least one year of experience equivalent to the next lower grade level; PGY1 residency or equivalent experience is preferred. The position is Monday-Friday, 8:00 a.m. to 4:30 p.m., with no telework or virtual work.
