Description
The Hospital Pharmacist provides comprehensive pharmaceutical care in a hospital setting, including medication-order review, drug-interaction monitoring, medication therapy management, patient counseling, medication reconciliation, IV and chemotherapy preparation, pharmacy operations, medication-safety monitoring, and interdisciplinary collaboration. The role requires a California pharmacist license, ACLS and AHA BLS certification, and either three years of hospital pharmacy experience or one year of PGY1 pharmacy residency; PGY-2 residency completion is preferred. The position offers variable shifts, including every-other weekends, and pays $77.910 to $112.590 per hour.
