The Hospital Pharmacist provides comprehensive pharmaceutical care in a hospital pharmacy, including clinical consultations, medication therapy review, chemotherapy preparation and monitoring, IV admixture, dispensing, patient counseling, drug information, medication safety, regulatory compliance, and interdepartmental collaboration. The role requires a California pharmacist license, AHA BLS Healthcare, and ACLS certification, with hospital pharmacy experience preferred.
