Community Physicians is hiring a Nurse Practitioner or Physician Assistant for a full-time, W-2 Skilled Nursing Facility PM&R program in Aurora, Illinois. The role provides comprehensive physical medicine and rehabilitation care to post-acute and long-term care residents, including stroke recovery, orthopedic and spinal conditions, chronic pain, and mobility disorders. The provider participates in structured daily rounding, follows standardized clinical pathways, coordinates care with interdisciplinary teams, supports discharge planning, and receives mentorship and didactic education from a board-certified physiatrist. The position requires an active Illinois NP or PA license and national certification, with 1–3+ years of relevant experience preferred, and offers a $120,000 salary plus performance bonus and benefits.
