Community Physicians is hiring a full-time Nurse Practitioner or Physician Assistant for its Skilled Nursing Facility Physiatry Program in Aurora, Illinois. The role provides comprehensive physical medicine and rehabilitation care to post-acute and long-term care residents, including stroke recovery, orthopedic and spinal conditions, chronic pain, and mobility disorders. The provider participates in structured daily rounding, follows standardized clinical pathways, coordinates care with interdisciplinary teams, supports discharge planning, and works under the mentorship of a board-certified physiatrist. The position requires an active Illinois NP or PA license and national certification, with 1–3+ years of relevant experience preferred, and offers a $120,000 salary plus performance bonus and benefits.
