Community Physicians is hiring a full-time Nurse Practitioner or Physician Assistant for its Rockford, Illinois Skilled Nursing Facility Physiatry Program. The role provides comprehensive physical medicine and rehabilitation care to post-acute and long-term care residents, including stroke recovery, orthopedic and spinal conditions, chronic pain, and mobility disorders. The provider participates in structured daily rounding, follows standardized clinical pathways, coordinates care and discharge planning, and works with an experienced physiatrist and interdisciplinary team. The position requires an active Illinois NP or PA license and national certification, with 1–3+ years of relevant experience preferred, and offers $120,000 in salary plus a performance bonus and benefits.
