AdvisaCare is hiring licensed Physical Therapists to provide patient-centered care in home health and community settings in Michigan. The role includes evaluating patients, creating and modifying individualized treatment plans, delivering therapeutic exercises and interventions, documenting progress, educating patients and caregivers, and coordinating with interdisciplinary care teams.
