The position develops individualized treatment plans, implements therapeutic activities, educates patients and families, collaborates with healthcare professionals, monitors progress, documents care, and advocates for patients. Applicants must meet professional licensure and certification requirements, pass a background check, obtain a Level One Fingerprint Clearance Card, and be able to travel to patients’ homes.
