One Home Health is hiring a part-time Physical Therapist to provide patient-centered home health care in Valparaiso, Porter County, LaPorte County, and surrounding areas. The role includes physical therapy evaluations, treatment, care-plan development, documentation, interdisciplinary communication, patient and caregiver education, discharge planning, quality improvement, and supervision of Physical Therapist Assistants and home health aides. Candidates must graduate from an accredited Physical Therapy program, hold a current license in the state(s) of practice, have at least two years of healthcare experience, hold current CPR certification, and have a valid driver’s license with reliable transportation.
