The role involves evaluating patients in their homes, creating and delivering individualized treatment plans, providing therapeutic interventions such as exercise and gait training, educating patients and caregivers, documenting care digitally, and coordinating with an interdisciplinary clinical team. The position requires an active Texas PT license, professional liability insurance, and a graduate degree or higher in Physical Therapy; home health experience is preferred.
