Human Touch Home Health is hiring a Physical Therapist for PRN home health opportunities serving Quakertown, Coopersburg, and surrounding Upper Bucks County communities, with additional patient opportunities in Perkasie, Sellersville, Richlandtown, Milford Square, Trumbauersville, and nearby areas. The role involves home-based physical therapy evaluations, individualized plans of care, mobility and fall-risk training, patient and caregiver education, progress monitoring, discharge planning, care coordination, and WellSky documentation. An active Pennsylvania Physical Therapist License, accredited physical therapy education, a valid driver's license, reliable transportation, and strong clinical assessment and communication skills are required; home health and WellSky experience are preferred or advantageous.
