Description
The Physical Therapist Assistant will provide home-based rehabilitative care under the supervision of a Physical Therapist, implementing treatment plans, reinforcing home exercise programs, monitoring patient progress, coordinating with interdisciplinary teams, educating patients and caregivers, documenting clinical notes, and supporting agency operations. The role requires an associate’s degree in Physical Therapy, registration with the New Hampshire Board of Nursing, at least two years of hospital or clinic experience, CPR certification, and the ability to travel within the agency’s territory.
