Description
The company is hiring Nurse Practitioners to manage patients in skilled nursing and long-term care facilities. The role provides provider-level direct patient care, including annual wellness visits, episodic and re-evaluation visits, treatment of chronic and comorbid illnesses, medication and diagnostic review, care coordination with physicians and other disciplines, and communication with families. The position is on-site and available in full-time or part-time schedules within 8:00 AM–6:00 PM, and requires a Texas Physician Assistant License, an accredited Physician Assistant program graduate, nursing experience, a DEA License, and proof of COVID vaccination.
