Southeast Primary Care Partners is hiring a Population Health Transformation Specialist to lead the transition to outcomes-based and value-based care across multiple clinics. The role partners with providers, managers, and clinical staff; supports implementation of quality and care-gap-closure pathways; tracks HEDIS and STARS performance; reviews EHR documentation; provides training; and helps identify improvement opportunities. The position requires experience with value-based payer programs, HEDIS and STARS initiatives, electronic health records, clinical training, and audit processes, along with the ability to travel to multiple locations.
