Description
The Post Transition Service Supervisor supervises 5–7 HAY Center case managers and coordinators, ensures compliance with the Preparation for Adult Living contract and Council on Accreditation requirements, manages referrals and case files, maintains case-management and contract-reporting databases, prepares reports, reviews payment requests, and coordinates with community partners. The role requires a bachelor’s degree in a behavioral or social science field, at least two years of supervisory experience in social services, at least three years of experience working with transition-age youth, computer and case-management software skills, and the ability to become CPR certified. It is a full-time regular position with Monday–Friday hours and possible Saturdays, with employment contingent on a background check and drug screen.
