Urban Pathways is hiring a Licensed Practical Nurse to provide professional and clinical nursing care to clients in residences and programs. The role assesses and develops nursing care needs, coordinates medical, mental health, and community services, monitors medication and client behavior, supports discharge planning, manages crisis situations, and maintains client documentation. It is a hybrid position requiring four days in the office and one day remote, with field work, client escorts, and public transportation. The position requires graduation from an accredited practical nursing school, a New York State Licensed Practical Nurse license, and a flexible schedule including weekends, holidays, and evenings.
